Filled Miscellaneous application form
- 2 recent passport size photographs showing full frontal view of the person's head and shoulders having a white or light-colored background. No glare if wearing glasses.
- Original and self-attested copy of valid Indian Passport (first and last pages)
- Original and self-attested copy of valid Cyprus visa/ residence permit
- Request Letter stating the purpose of Life Certificate
- The issuance of the certificate is subject to verification of the Documents/credentials submitted by the applicant.
SUBMISSION OF APPLICATION FORMS:
